Disease management The diagnosis of a chronic illness presents individuals with a large challenge. The affected person’s life changes radically, lots of factors of uncertainty affect his/her state of health and often the person feels completely alone in many regards. Often these factors lead to an early end of therapy, which could be prevented with suitable measures.
With consultation, support and other measures, which are not directly connected with medical aspects of the illness, the general feeling of well-being can be increased and thereby the course of the illness can be decisively influenced.
An essential goal here is the improvement in the quality of life for long-term patients and their close family/friends, and the relief of all people involved in the therapy. A suitable consultation offer fills the gaps with regards to questions about individual and everyday life situations of long-term patients, and leads to a higher level of self-competence and greater trust in the therapy thanks to information being orientated to the patient.
